Community Health Improvement Plan (CHIP)
The Charlotte County Community Health Improvement Plan (CHIP) is developed through a collaboration of local residents, non-profit organizations, health care professionals, school representatives, and many other community members working on programs and plans to benefit all residents of Charlotte County. Known in the community as Healthy Charlotte County, we help make recommendations, develop, and implement community projects that improve health outcomes and create environments that support healthy living. Through our health improvement projects, Healthy Charlotte County aims to identify issues community members feel are important and find ways to solve some of our most pressing health issues.
Healthy Charlotte was created as a result of the Community Health Assessment (2015) (pdf 5.50 mb) and Community Health Improvement Plan 2016-2018 (pdf 1,837 kb). The Community Health Assessment helped Healthy Charlotte partners to identify the need for a Senior Health Assessment, conducted in 2017.
Currently, the group is in the process of developing a new Community Health Improvement Plan to cover 2019-2020.
