Florida birth certificates are issued through the Vital Statistics Office at the Florida Department of Health in Charlotte County (DOH-Charlotte).

In Person Orders:

For a certified copy of a birth certificate stop by our location listed below. 

The cost for a birth certificate is $12.00 for each certificate.  A legal photo ID is required.

*Please note: There is a service fee of $6.25 when obtaining certificate from Tax Collector’s Office. 

Orders by Mail:

To order by mail, send an application to the DOH-Charlotte office, as listed above. A copy of a legal photo ID is required along with the application, payment, and stamped self-addressed envelope.

Many companies not affiliated with the Florida Bureau of Vital Statistics have official looking websites and offer their services to order vital record certificates. If a website relating to vital record certificates does not contain .gov in it's web address, it is NOT an official government website.
