The Vital Statistics Office at the Florida Department of Health in Charlotte County issues death certificates for deaths that occurred in the State of Florida from 2009 to present. Orders may be placed in person Mon-Fri 8 a.m. to 4 p.m., by mail, or online through vitalchek.com. Death certificate fees are $10.00 per copy - Cash, Credit Cards, Check or Money Orders payable to “CCHD” accepted.

Florida Department of Health in Charlotte does not accept credit cards for online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network, Inc. VitalChek can be reached either through its website, www.vitalchek.com, or by phone at 1-800-255-2414.  An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.

A convenience fee of $7.00 is charged.

These certificates are issued in the following two formats:

  1. Certified copies with cause of death listed. These copies have restrictions on issuance.
  2. Certified copies without the cause of death listed. No restrictions on issuance.

Florida Death Certificates Can Be Obtained By: 

  1. Immediate family members
  2. Attorney(s) representing immediate family members.
  3. Banks, executors of estates, insurance companies or anyone requiring a death certificate to pay a policy or death benefit on the decedent.
