Practitioners must complete this form to notify DOH-Charlotte of a reportable disease or condition.

The Florida Department of Health in Charlotte County (DOH-Charlotte) is responsible for the surveillance, investigation, control, and prevention of infectious diseases of public health importance in Charlotte County, Florida, in accordance with Florida Administrative Code 64D-3.

DOH-Charlotte's Infectious Disease Services include:

  • Epidemiology Program- DOH-Charlotte's Epidemiology Program performs surveillance and investigates cases of reportable diseases and conditions.
  • Health Care-Associated Infection Prevention Program (HAI) - DOH-Charlotte's HAI Program is a non-regulatory, grant-based program developed to assist health care facilities with the implementation and maintenance of infection prevention practices and policies.
  • Rabies Prevention ProgramDOH-Charlotte's Rabies Program consults with patients, physicians, and veterinarians regarding the current recommendations for treating a rabies exposure.
