Florida certificates are issued through the Bureau of Vital Statistics at the Department of Health. Orders may be placed online or by phone through VitalChek®, by mail, or in-person. VitalChek® is the only vendor recommended by the Florida Department of Health. Fees charged by VitalChek® may be different than fees charged when ordering directly through your local health department office.

Birth Certificates
Florida birth certificates are available through Florida Department of Health in Charlotte County’s (DOH-Charlotte) Vital Statistics Office, Florida Department of Health’s Bureau of Vital Statistics, and VitalChek®.
Death Certificates
Florida death certificates for 2009 to present are available through our Vital Statistics Office. Death records before 2009 may be obtained from the Florida Department of Health’s Bureau of Vital Statistics and VitalChek®.
Marriage Certificates
Florida marriage certificates from June 6, 1927, to present that have been recorded by the Clerk of Circuit Court are available through Florida Department of Health’s Bureau of Vital Statistics and VitalChek®.
Divorce Certificates
Florida dissolution of marriage certificates from June 6, 1927, to present that have been recorded by the Clerk of Circuit Court are available through Florida Department of Health’s Bureau of Vital Statistics and VitalChek®.
