DOH-Charlotte's Vital Statistics Office Hours: Monday - Friday, 8 a.m. - 4 p.m.
Clients are encouraged to call DOH-Charlotte ahead of their visit.
Florida birth certificates are available through Florida Department of Health in Charlotte County's (DOH-Charlotte) Vital Statistics Office, Florida Department of Health's Bureau of Vital Statistics, and VitalChek®.

Orders by Mail
To order by mail, complete and mail the Application for a Florida Birth Record to DOH-Charlotte. A copy of a legal photo ID is required along with the application, payment, and stamped self-addressed envelope.

The cost for a birth certificate is $12. A legal photo ID is required.
