Clients are encouraged to call DOH-Charlotte ahead of their visit.
Florida death certificates for deaths that occurred in Florida from 2009 to present are available through Florida Department of Health in Charlotte County's (DOH-Charlotte) Vital Statistics Office. Death records before 2009 may be obtained from the Florida Department of Health's Bureau of Vital Statistics. Death certificates are also available through VitalChek®.
Death Certificates are issued in two formats:
- Certified copies with cause of death listed (long form). These copies have restrictions on issuance and can be obtained by:
- Immediate family members
- Attorney(s) representing immediate family members
- Banks, executors of estates, insurance companies, or anyone requiring a death certificate to a pay a policy or death benefit on the decedent
- Certified copies without the cause of death (short form). No restrictions on issuance.

Orders by Mail
To order a death certificate by mail, complete and send an Application for a Florida Death Record to DOH-Charlotte's Main Office office.

A copy of a legal photo ID is required with the application, payment, and stamped self-addressed envelope.

Florida Department of Health's Bureau of Vital Statistics offers walk-in or mail-in death certificate services. Normal processing time for death certificates from 2009 to present is three to five business days (not including shipping time). Records prior to 2009 require additional processing time.
