Community Health Needs Assessment (CHNA) Survey

Charlotte County, Fla.— The Florida Department of Health in Charlotte County (DOH-Charlotte) needs your voice! The Community Health Needs Assessment (CHNA) survey has been released to the public and DOH-Charlotte is asking all community members to provide their input. Participants can enter the drawing for a $100 Visa gift card after completing the survey.
DOH-Charlotte is currently in the process of completing their 2025 CHNA; a process that is conducted every five years. The 2025 CHNA is being produced in partnership with Area Agency on Aging for Southwest Florida, Charlotte Behavioral Health Care, Charlotte County Community Services Department, Charlotte County Human Services Department, Drug Free Charlotte County, Englewood Community Coalition, Gulf Coast Partnership, the Health Planning Council of Southwest Florida (HPC), United Way of Charlotte County, and Virginia B. Andes Volunteer Community Clinic.
The CHNA utilizes the Mobilizing for Action through Planning and Partnership (MAPP 2.0) framework developed by the National Association of County and City Health Officials (NACCHO). This framework helps communities prioritize public health and well-being issues, identify resources for addressing them, and take action to improve conditions that support healthy living.

The survey results will be used to assess the overall health and well-being status of Charlotte County’s community. The survey contains questions such as, the availability of various health and community services, and are based primarily on the perception and opinion of the public. Primarily, the survey asks general questions about the most important health and wellness concerns, the requirements for a healthier lifestyle, and where residents get their healthcare.
The survey consists of 43 questions and contains multiple-choice and Likert scale questions. It can be completed in approximately 10 minutes. The survey will be available through October 4, 2024. All community members are welcomed and encouraged to complete the survey either electronically or on paper.
