Community Health Needs Assessment Survey

Port Charlotte, Fla. — The Department of Health (DOH) in Charlotte County needs your voice! The Community Health Needs Assessment (CHNA) Survey has been released to the public. DOH is asking all community members to complete the survey.

DOH-Charlotte is in the process of completing their 2020 Community Health Needs Assessment; a process that is conducted every five years. The 2020 CHNA is being produced in partnership with United Way of Charlotte County, Charlotte County Human Services Department, Charlotte Behavioral Health Care, and the Health Planning Council of Southwest Florida (HPC). The Community Health Needs Assessment utilizes the Mobilizing for Action through Planning and Partnership (MAPP) framework developed by the National Association of County and City Health Officials (NACCHO) in partnership with the Centers for Disease Control and Prevention (CDC). This framework helps communities prioritize public health and well-being issues, identify resources for addressing them, and take action to improve conditions that support healthy living.

“This process offers an opportunity for our community members to have a voice in the health of Charlotte County and allows them the opportunity to be proactive in planning for future health and well- being improvement initiatives.” Joe Pepe, Interim Administrator.

The community survey will be used to assess the overall health and well-being status of the Charlotte County community. The survey contains questions such as, the availability of various health and community services. Questions are based primarily on the perception and opinion of the public.

Primarily, the survey asks general questions about the most important health and wellness concerns, the requirements for a healthier lifestyle, and where residents get their healthcare.

The survey consists of 40 questions, of which, 16 are demographic questions. The survey contains multiple-choice and Likert scale questions. It can be completed in about 10 minutes.

The survey will be available through November 22, 2019. All community members are welcomed and encouraged to complete the survey either electronically or on paper. It is being offered in English as well as in Spanish.

All surveys are anonymous.

In addition, all Charlotte County residents are invited to attend a community health and well-being strategic issues prioritization meeting on December 12, 2019 from 9:00 a.m. to 12:00 p.m. at the Department of Health located at 1100 Loveland Blvd, Port Charlotte FL 33980.
