Community Health Planning and Statistics conducts ongoing community health data collection and analysis in an effort to understand and monitor the health status and health needs of residents of Charlotte County. 

Community Health Status

A community health assessment is the systematic collecting and analyzing of data to educate and mobilize communities, develop priorities, garner resources, and plan actions to improve public health. Through data available in Florida Charts, the Florida Department of Health in Charlotte County (DOH-Charlotte) is able to provide the analysis of data for the assessment process.
