Community Health Needs Assessment (CHNA) Survey

Charlotte County, Fla.— The Florida Department of Health in Charlotte County (DOH-Charlotte), in conjunction with Healthy Charlotte, encourages all Charlotte County residents to complete the 2025 Community Health Needs Assessment (CHNA) Survey. The survey is available in English, Spanish, Haitian Creole, and Russian. The survey is anonymous and open until October 4, 2024.  

Participants can enter a drawing for a $100 Visa gift card. The survey consists of general questions about the most important health and wellness concerns, the requirements for a healthier lifestyle, and where residents get their health care. It takes approximately 10 minutes to complete. The survey results will be used to assess the overall health and well-being status of Charlotte County residents.
