Kindergarten Readiness

School Physicals

All students will need a school physical before starting Kindergarten.  A school physical can be obtained by seeing your child'spediatrician or at a local walk-in clinic.The Florida Department of Health in Charlotte County does not provide school physicals..

Birth Certificates

Parents will need an original birth certificate.

Eligibility:

Parent/Guardian will need a valid government issued id to obtain the certificates.

Birth Certificates can be issued only to:

  1. Registrant (the child named on the record) if of legal age (18)
  2. Parent(s) listed on the Birth Record
  3. Legal guardian (must provide guardianship papers)
  4. Legal representative of one of the above persons (must provide proof)
  5. Other person(s) by court order (must provide recorded or certified copy of court order)


For children born in the State of Florida

The parent can go to any county health department to obtain the record.

They do not need to go to the county the child was born

Times and hours vary by county.

 

In Charlotte County there are two locations to obtain a birth certificate.

Certificates are $12 each.

Fees vary by state

 

Immunizations

Immunization records will need to be on a Florida 680 form.

Most children going into kindergarten need the 4-6 year old vaccines.

The Department of Health in Charlotte County offers appointments for children for vaccines.

Parents are encouraged to call early to set up their appointments.

Will need to bring:

  • Previous shot records
  • Child’s social security card
  • Parent will need a valid government issued ID.
