The Charlotte County Community Health Improvement Plan (CHIP) is developed through a collaboration of local residents, non-profit organizations, health care professionals, school representatives, and many other community members working on programs and plans to benefit all residents of Charlotte County.  Known in the community as Healthy Charlotte, we help make recommendations, develop, and implement community projects that improve health outcomes and create environments that support healthy living.  Through our health improvement projects, Healthy Charlotte aims to identify issues community members feel are important and find ways to solve some of our most pressing health issues. 

Healthy Charlotte was created as a result of the Community Health Assessment (2015) (pdf 5.50 mb) and Charlotte County Health Improvement Plan 2019-2020 (pdf 4,608 kb).  The Community Health Assessment helped Healthy Charlotte partners to identify the need for a Senior Health Assessment, conducted in 2017.
