School, Sport, and College Physicals
Students from prekindergarten through 12th grade who are making their initial entry into a Florida school must present a record of a school entry health examination completed within one year before enrollment.
A form signed by a health care provider licensed to perform physical exams in the U.S. that indicates the results of the components included in School Entry Health Exam form (DH3040-CHP-07/2013) is also acceptable.
Florida law also requires electrocardiograms (EKGs) screenings for student athletes in public high schools. At DOH-Charlotte, sports physicals include EKG screenings.
If your children need a health exam or physical to start school, join a high school sports team, or enroll in college, call DOH-Charlotte at 941-624-7200.

We also offer children’s immunizations, dental services for children under 21, car seat installations, and WIC nutrition services.

Additional services for women include Papanicolaou (Pap) smears, human papillomavirus (HPV) testing, Women, Infants, and Children (WIC), and the Florida Breast and Cervical Cancer Early Detection Program.
We also have a fall prevention program for older adults with concerns about falling.
